Probably the PSU if board and CPU are fine...or the connections from the PSU

If the board supports it "BIOS Flashback" method is the most reliable ....followed by EZ Flash utility in BIOS

I would start there, yes. Try changing DRAM voltage to 1.35v and maybe 1.4 or 1.45..this is totally safe voltage for RAM

OCing these GPUs can draw well over 350W alone...with an OCd CPU and important headroom...you want to get a PSU that will be in the sweet spot of power delivery...not near its maximum. A good quality...
